Appearance and Waiver of Service Form Uncontested Dissolutions and Entry of Appearance and Waiver of Service Form (1-02)It is hereby ordered that administrative order 18-97 is repealed. It is further order that, in uncontested dissolution cases all "entry of appearance and waiver of service" forms shall contain a notice to the respondent that the respondent is to attend the Focus on Kids parent education program within thirty (30) days, if so required by Local Court Rule 68.6. The "entry of appearance and waiver of service" form, when filed with the court, shall also include the respondent's address and telephone number. Ordered: February 4, 2002 Preparation of Subpoenas for Witnesses (6-02)It is hereby ordered that administration order 11-92 is repealed. It is further ordered that the Circuit Clerks' office shall provide subpoena forms in sufficient quantity for distribution to the parties. In all matters, the subpoenas for witnesses shall be completed by the requesting party by inserting the name of the court, title of the proceedings, and time and place for the person being subpoenaed to appear. The clerk shall then issue the subpoena under the seal of the court or, upon request of the party, the clerk shall issue a subpoena, signed and sealed but otherwise in blank, to a party who shall fill in the blank before it is served. The party shall provide the clerk with a file copy of the subpoena upon completion. Ordered: February 4, 2002 |
