Party Information SheetsAt its February 7, 2005 meeting, the 13th Judicial Circuit Court en Banc approved the use of "Party Information Sheets" designed by the Office of State Courts Administrator (OSCA) to comply with Supreme Court Operating Rule 4.07.01. Following are links to the Party Information Sheets as designed by OCSA in Microsoft Word format (they are designed to be typed on a typewriter rather than filled out in a word processor):
Following are links to PDF versions of the Party Information Sheets as designed by OSCA. The PDF versions also are not designed to be filled out on the computer and must be printed and typed on a typewriter.
Following are links to Party Information Sheets designed by Steve Scott in both Microsoft Word and WordPerfect formats which can be filled out on the computer. Note that these Word and WordPerfect forms use tables and that locations where information is typed are marked with the "^" symbol. Using the "repeat search" feature in your word processor, you can quickly move to the next location where information is typed. If you prefer, you can delete the symbols or use another symbol. Also, note that in both Word and WordPerfect, it is possible to quickly move from one table cell to the next by pressing the tab key.
IMPORTANT NOTE: The Party Information Sheets request a "case type." OSCA has promulgated lists of case types to go with each type of Party Information Sheet. On the pre-printed versions of the Party Information Sheets available at the Circuit Clerk's office, the case-type lists appear on the back sides of the forms. Note that the Circuit Civil and Circuit Family Court lists appear identical at first glance, but are clearly different upon closer study. The case-type lists from OSCA appear below in both Word and WordPerfect formats:
